OUR PHILOSOPHIES (Working Principles): S.T.E.P.S

The following are the five main philosophies that underpin the work we do:

  1. SELF-CARE: Health is a gift and most people have appreciable level of responsibility and capability to maintain the gift of health & life.  Self-management is the term given to a range of approaches that enable people to manage their own health effectively. Self-management principle offers that
  2. people are, to a large extent, experts of their own lives, and
  3. when people have the necessary skills and knowledge for managing their life and health, they often help in achieving better healthcare outcomes, patient experience, reduced unplanned hospital admissions and improved adherence to treatment.
  4. TARGETED-CARE: Excellent professional healthcare is an intentional and planned venture. Patients' experience and treatment outcome are, hence, pivotal in our care-plan from start.
  5. EVERYONE-MATTERS: Everyone deserves high quality professional healthcare (mental and physical) because good health is a basic human right and mental health is an integral part of our overall health.
  6. PERSONAL CHOICES . We believe that clients are experts of their own lived experiences and that their appropriate choices are vital in achieving life potentials and sustaining optimal health. We encourage clients to be involved in their health and care, so that they can experience better quality care and improved health & wellbeing. We believe that people’s lives and health can be transformed when:
  7. they feel in control of their health and wellbeing and
  8. they are able to contribute to their care-plan, support and treatment. When people are involved in decisions about their health and care, they tend to choose healthcare packages that align with their personal preferences and goals.
  9. SOCIALISATION OF HEALTH (Community-based Care): Social networking and support promote public health & individual wellbeing. Maintaining public health is a collaborative team efforts involving patients, families/friends, local authorities/government, and healthcare professionals etc. Person and community-centred approaches, therefore, support people to self manage, help to increase knowledge, skills and confidence to manage their health and wellbeing, improve outcomes, and reduce social isolation and loneliness.

OUR CORE VALUES: PREM - Q

The following are our Core Values:

  1. Prioritizing Clients' Needs
  2. Respecting Clients' Dignity & Privacy
  3. Embodying Excellence & Integrity
  4. Motivating Change & Sustainable Development
  5. Quality-driven Healthcare
  1. Prioritizing Clients' Needs
  2. When planning and implementing our clients' therapeutic care, we ensure that their needs and wishes are prioritised and their perspectives included in the decision-making process. This ensures that every client receives a unique and personalised healthcare.
  3. We therefore incorporate the following (VIP-S) (Brooker, 2015):
  • V = we value and promote the rights and choices of our clients. We value them as human being who deserve healthcare irrespective of age, class, culture, background and/or cognitive ability. This is the foundation for our personalised Healthcare
  • I = Individual’s needs (Personalisation of care): we provide individualised/personalised healthcare according to our clients' distinctiveness and holistic needs.  The individualised care conversations focus on what matters most to the clients (their personal goals) and the support they need to manage their health and wellbeing. It is a process of sharing information, identifying clinical support needs, discussing options, contingency planning, setting goals, documenting the discussion (often in the form of a care plan) and monitoring progress through regular review.
  • P = Perspective of our clients. Implementing our 'Partnership-in-Practice', we aim to see our clients' world from their perspective. We ensure that we collaborate and take the clients' point of view into account when planning and providing their healthcare.
  • S = Social Support.  We consider clients' social environment and how that supports the clients' wholistic needs (e.g., physical, mental/psychological, emotional and spiritual needs).
